Please do not include medical history, diagnoses, or other health details in any web form. Our team gathers any sensitive information securely by phone, in person, or through our care systems.
How we handle PHI elsewhere
In the course of providing services, Bella Behavioral Health maintains protected health information in accordance with the Health Insurance Portability and Accountability Act (HIPAA). A full Notice of Privacy Practices is provided to individuals receiving services and is available from our office.
